What I am looking to do is if the selected item in a combo box changes, the patient object will be updated through two-way binding as with the text controls. I am new to WPF and binding, and with windows forms, I would have trapped the selection changed event and made the change in there.

The UpdateSourceTrigger property of a binding controls how and when a changed value is sent back to the source. However, since WPF is pretty good at controlling this for you, the default value should suffice for most cases, where you will get the best mix of a constantly updated UI and good performance.

This is because we want to bind to properties that are located in the code-behind, which means it needs some way to reference itself. I was able to use some xaml to get the underlying ComboBox to virtualize so the load times are managable, but I havent been able to get the IsTextSearchEnabled property to work.
I ran into what looks like the same issue. I made sure my SelectedValuePath and DisplayValuePath are set correctly. The combo box is filled with the correct objects and values. But, If I try to do a two-way binding to the SelectedValue property, a null value is passed by the control and sets the value of the target object to null.
